Depending on the service, assessments may be completed by our experienced nurses or GPs, working together to deliver high-quality, comprehensive support.

Chronic Disease Management Plan

A Chronic Disease Management (CDM) Plan is designed for patients with a long-term medical condition such as diabetes, asthma, arthritis, or heart disease. It helps coordinate ongoing care with your GP and may include referrals to allied health professionals like physiotherapists or dietitians. These plans are reviewed regularly to ensure your treatment stays on track and is Medicare-supported.

Care Plan

Care Plans are personalised health management plans for patients with complex or multiple medical conditions. Developed collaboratively with your doctor and nurse, they outline goals, treatments, and support services to improve your health outcomes. They also provide access to subsidised allied health services under Medicare.

Over 75-Year-Old Home Assessment

A comprehensive health assessment conducted in the comfort of your home for patients aged 75 and older. It reviews physical, mental, and social wellbeing, medication use, home safety, and mobility. The goal is to support independence, identify any risk factors, and ensure you have access to the care you need.

45-49 Year Old Health Check

This preventative check-up is for patients aged 45-49 who are at risk of developing chronic conditions. It includes a full health assessment, lifestyle review, and risk screening for issues like heart disease, diabetes, and mental health. Early intervention can significantly reduce the risk of future health problems.

Aboriginal and Torres Strait Islander (ATSI) Yearly Health Assessment

An annual health check tailored for Aboriginal and Torres Strait Islander patients of all ages. It focuses on early detection and prevention of chronic disease, and supports culturally appropriate, holistic care. These assessments are fully bulk billed and can help you access further allied health or specialist services.

Newborn Health Check

Performed shortly after birth and during early GP visits, the newborn health check assesses your baby’s growth, development, reflexes, and feeding. It also provides a chance to discuss sleep, vaccinations, and any early concerns with your doctor or nurse.

Pre-School Check

A comprehensive health assessment typically performed around age 4, before starting school. It includes checks of hearing, vision, growth, speech, motor skills, and emotional development. This check helps identify any concerns early and ensures your child is developmentally ready for school.
