As part of Australia’s push to improve patient access and engagement in their own care, Services Australia has introduced a new feature allowing patients to view their care plan history via their Medicare account on myGov, effective 10 May 2025.
At Morphett Vale Family Practice (MVFP), we welcome this update and are ready to assist our patients in understanding their care plans and using the information to better manage their health, particularly for those living with chronic or complex conditions.
What Is a Care Plan?
Care plans are created by your GP to help manage chronic or complex medical conditions. These plans:
Outline your health goals and treatment steps
Coordinate care across healthcare providers
Include referrals to allied health professionals like physiotherapists, dietitians, and psychologists
At MVFP, we routinely develop and review these plans with you to ensure they remain relevant and supportive.
Accessing Your Care Plan History Online
Patients can now securely view their care plan history, including:
Dates plans were created or reviewed
What services have been accessed through the plan
The status of allied health referrals
This tool offers transparency and supports you in being actively involved in your healthcare.
How to Access Your Care Plan History
Log in to your myGov account
Visit my.gov.au and sign in.Ensure Medicare is linked
If you haven’t linked Medicare, click “Link a service” and select Medicare.Click on Medicare
This opens the Medicare online services portal.Go to ‘History and statements’ > ‘My care plans’
You’ll find a list of current and previous care plans, as well as referral activity.
Please note:
You can also view care plan history for children under 14 on your Medicare card.
Due to privacy regulations, individuals aged 14-17 will need to access their own Medicare accounts to view their care plans.
